WLS Welcome Card Registration

The WLS Welcome Card is a six-month temporary library card.  Those navigating re-entry after incarceration are offered this card as a way to assure access to library services.  The card is activated (and can be renewed) upon request.  The card can be replaced with a permanent card at any library for which the required identification can be produced, which is typically two forms of identification that can confirm your address.

You have likely reached this site because you received one of our Welcome Back brochures, which includes a card ready for activation.  To register a WLS Welcome Card, please complete the the appropriate form linked below.  You will receive a confirming email when the card has been activated.  This is typically done within 1 business day.
